Register with us as a New Patient

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.

You will need to complete a registration form (HS200) before your registration can be initiated, please see link below which you can print and complete and bring with you to the Practice Reception.

New patients who have existing long term conditions can book for a health check with our Practice Nurse to ensure that any required tests are up to date.

Please speak to Reception for assistance. Please be advised when signing documents to become an Organ Donor patients must be 18 or over. 

When presenting to register you will need the following:

  • HS200 for NORTHERN IRELAND & UK Residents ONLY.  This form must be completed if you wish to register with our practice. You will also be required to supply Photographic ID & Proof of a current Northern Ireland address. (Please refer to our registration policy below).                             
  • HSC/R1 –  This form is used for individuals entering Northern Ireland coming from any another country OUTSIDE the UK.  You will also be required to supply Photographic ID & Proof of a current Northern Ireland address. Any person registering from outside the EU must submit a copy of their Visa or Work Permit. 
  • New patient questionnaire & New Patient Agreement must both be completed.

Practice Area – 4.5 mile radius

The practice operates within a 4.5 mile radius.  The practice will retain patients currently on the list who reside outside the new boundary until such times as the patient moves to another area. (25.10.19)

Registering your baby

You need to fill in a GRO 4 Birth Registration Form. The General Registry Office will give you this when you register your baby’s birth. Bring this form to the GP’s surgery to register your baby as a patient.

From Feb 2016 the Practice is part of a pilot scheme with a new style form Blue HS22x for all patients to complete, more identification documents may be required when you present at the reception desk.
