New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.
You will need to complete a registration form (HS200) before your registration can be initiated, please see link below which you can print and complete and bring with you to the Practice Reception.
All new patients are offered a health check with our Practice Nurse to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking. Please remember to bring a urine sample & your New Patient Registration Form with you to your New Patient Registration Appointment.
Medical treatment is available from the date of registration, not the date of your New Patient Registration Appointment. Please speak to Reception for assistance.

Practice Area – 4.5 mile radius
The practice operates within a 4.5 mile radius. The practice will retain patients currently on the list who reside outside the new boundary until such times as the patient moves to another area. (25.10.19)
